Mr. Edwardo Carrasco, Dallas, TX; Case Number B–36613
Violation: Mr. Carrasco offered and practiced engineering without being licensed by the Board and falsely signed and sealed engineering documents indicating a professional engineer performed or directly supervised the work on fifteen Texas projects.
Act/Rule Violated: Tex. Occ. Code §§ 1001.004(c)(2)(A), 1001.301(a) and (c)–(e), and 1001.405(b)(1)–(2) and 22 TAC §137.37(b).
Resolution: Cease and Desist Order and Administrative Penalty of $35,700.00.

Bijay Raj Aryal, P.E. #109689, Fulshear, TX; Case Number D–37414
Violation: Mr. Aryal failed to practice engineering in a careful and diligent manner by issuing a misleading engineering design sheet. Mr. Aryal failed to properly and promptly notify a licensee after he accepted an assignment which may impact the licensee's original engineering plans. Mr. Aryal neglected to include an execution date on an engineering document.
Act/Rule Violated: 22 TAC §§ 137.33(f), (i)(1), and (n), 137.37(a)(3), 137.57(a), 137.57(b)(3), and 137.63(b)(6).
Resolution: Three Year Probated Suspension contingent on remittance of an Administrative Penalty of $4,185.00 and successful completion of Texas Tech University Engineering Ethics Basic Course.

Jeremy Alan Smith, P.E. #107749, Forney, TX; Case Number D–37554
Violation: Mr. Smith failed to complete 15 Professional Development Hours (PDH) of continuing education for the 2017 renewal period and misrepresented he completed 15 PDH of continuing education for the 2017 renewal period. Mr. Smith failed to reply to Board’s request for information.
Act/Rule Violated: 22 TAC §§ 137.17(a), (c), (d), (j), (m), (p)(2), and (p)(3), 137.51(c), 137.57(a)(1)–(3), and 137.63(a).
Resolution: Two Year Probated Suspension contingent on remittance of an Administrative Penalty of $4,000.00.

Mr. Nick Manesh, P.E. #60994, Dallas, TX; Case Number D–37549
Violation: Mr. Manesh failed to timely provide plans and/or specifications to the Texas Department of Licensing and Regulation (TDLR) as required by Texas Government Code §§ 469.102(a) and (b). Mr. Manesh failed to ensure the design plans were submitted to TDLR no later than the 20th day after the date the plans were initially issued.
Act/Rule Violated: Tex. Occ. Code §1001.452(5) and 22 TAC §§ 137.63(b)(1) and (b)(2).
Resolution: Formal Reprimand and Administrative Penalty of $900.00.
